Put simply, the active ingredients are mixed with the glue that holds the patch in place.
Evidently it's not quite as simplistic as that, the mixture has to be kept consistent throughout the manufacturing process which is achieved by making smaller batches which ensures an equal spread of the ingredients over all the patches.
